I would like to add a few fields for submitting tickets for my clients. Found that it could not be done, and wanted to suggest this for future releases.

I'm pretty sure this can already be done. Check your config options again.
This can be done when setting up the departments or editing them. Take a look in Configuration > Support Departments.
